Internship Forms

A student who wishes to enroll in the HON 484 course for credit must submit three documents to receive the course SLN. Those documents can all be found on this page, and include the Affiliation Agreement, Student Information Sheet, and Book Recommendation Form. Please deliver the completed forms to Best C Hall Room 128 or fax, attention: Jill Johnson, to 480-965-0760.

All Barrett students who engage in an internship, regardless of whether they enroll in the HON 484 Internship Course for credit, must be sure that their company/organization submits an Affiliation Agreement.
